the english worked fine, but it requested my gmail account credential to open it.

it will not without creating an account. Easiest way would be to put document on server as previously ( last yr) or to make pdf form. where acrobat reader can take care of it by itself

English version of registration Form

When I try to access the French version, I get..Sorry, we are unable to retrieve the document for viewing or you don't have permission to view the document.
