TC,
There is no one specifically that the note is made out to. I work for a very large school board and they have several people who scrutinize the "sick days". They actually inplemented a program called PRISM (can't remember what it stands for anymore) where they keep track of your sick days, when you use them, how long you have been off work, etc., and then they contact you suggesting that you might want to see the "board doctor" to confirm your illness or look at ways to keep you healthy (aka....at work more often!!!!). I was in a car accident and off work for three weeks and I had to get medically cleared to return to work by their doctor (now any work related injury to my back will be claimed as a pre-known condition and not qualify for compensation!!!).
Anyway, the note needs only to say that I will be off work for medical reasons for the specified time period. Other doctors don't even address them, but if you want " To whom it may concern" is goog enough for me!!
